How to Order Medical or Office Supplies for the Isolation Unit
All requests must include the “Order Supplies Form”.
There is a separate tab for medical supplies and office supplies.
To submit a request:
Print and complete the appropriate Order Supplies form. The Nu-Life Medical & Surgical Supplies Catalog is located here.
Email the completed form to kirwin@kdchc.org
Once receive the request will be reviewed and you will be notified of the status of your request.
Please note:
Medical supplies will be shipped to Kitchener Downtown Community Health Centre and will need to be picked up once they have arrived at the centre.
You will be notified when they are available for pick up.
Before requesting any prescription medications,
confirm that the client does not have any drug benefits.
Kitchener Downtown CHC does not have drug benefit funding for prescriptions.
Some stock supplies can be ordered.
