The primary health care team and community health workers partner with community agencies to offer support to people who may find it difficult to access programs and services at the Centre. These outreach sites include:
St John’s Kitchen: This service is for people who use the services of the Kitchen. Our team at this site includes a Nurse Practitioner (for episodic issues), a Chiropodist, and a Community Health Worker who provides Identification services for those who are homeless/at risk. The Outreach clinic is held on Tuesday from 10-1.
Anselma House: A Nurse Practitioner provides episodic care for residents of the Shelter once per week.
oneROOF (Reaching our Outdoor Friends): A Nurse Practitioner provides episodic care for those who access ROOF services. This service is offered Wednesdays from 2-4.
