Community Healthcaring Kitchener-Waterloo provides primary care services, including illness prevention, health promotion and health care support to clients who are rostered with us. 

 

We take pride in providing client centred care, working with clients towards their identified goals so that people may strive for good health.  In addition to the care and support we offer; we also assist clients in accessing community services and supports.

 

We focus on the whole person, looking at all the issues that may affect a person. These issues are called the social influences of health, some of which are housing, income, social support, education, food security, environmental issues, and social justice.

Our Services

 

Community Healthcaring Kitchener-Waterloo is not a walk-in clinic. The Health Centre does provide some same day appointments. The following services are provided for rostered clients of the Centre. 

 

Our Primary Care team consists of Physicians, Nurse Practitioners and Nurses. Clients see the most appropriate provider for their health issues and concerns. View a full list of our providers/resources here.  You many view the Client Handbook here

 

Services include diagnosis, treatment, education, referral, and follow-up on health issues. Some of the specific services are complete health check-ups, well-baby checks, and immunizations. These are provided on-site at 44 Francis St. South to rostered clients only. View information for Refugee Health (provided at the 310 King St. East location) here.
